| 1. | Identify the Key Variables you would like
to collect, analyze, report or
graph. This can be in the
form of a handwritten
list, an email, a worksheet, a word processing document, a photocopy of
existing data collection forms, sample test results or a combination of
all the above. |

set-up a short meeting (about
30 minutes). Your
list and needs
will be reviewed with the database developer Gloria who is also well
versed
in medical terminology and healthcare processes. |
| 3. | A negotiable and
affordable Quote will be provided. With
your approval, the
work will begin off site so that there is no interruption to your
practice. In
approximately
4-6 weeks, a time will be booked to deliver the finished result ready
to go! |
